    your device E250S:
    GSM850, GSM900, GSM1800, GSM1900
    UMTS1900 (B2), UMTS2100 (B1)
    LTE850 (B5), LTE1800 (B3)

    your carrier Smart PH,Philippines :
    850 (b5 )/ 2100 (b1)

so no problems between the carrier and device
    but sonething is strange :

    original CSC code: NEE ( - Nordic (Halebop, Aina,Spring,Telenor,NetCom,Mobile Norway,Tele2 No,DNA,Elisa,FIN SONERA,Saunalahti,Telia,DK TDC,CallMe &amp DLG,Telenor,Telmore,Tele2comviq,3,Nova,Vodafone Iceland,Siminn,Tele Greenland,Ventelo) ? there is no NEE firmware for this device

    product code: Not Active

    you can try this:
    1: save all your data
2: factory reset with the keyboard dial : *#87976633#
3: recovery mode for wiping cache partition and data/factory reset [ Login above or register to see download links. ]

    regards

Does factory resetting via dialler makes any difference. I did factory reset my phone several times while installing different roms.

yes factory resetting via dialler can make differences but i think your problem is with the modem and CSC files ( your device is a mix with E250S and N7100 files or a custom rom )
    original firmware:
    Model: SHV-E250S
    Model name: GALAXY Note II
    Country: South Korea
    Version: Android 4.4.2
    Changelist: 2857324
    Build date: Mon, 29 Sep 2014 06:37:17 +0000
    Product Code: SKT
    PDA: E250SKSUKNI2
    CSC: E250SSKTKNI2
    MODEM: E250SKSUKNI2
[ Login above or register to see download links. ]
    so after :
    1: saving all your data
2: factory reset with the keyboard dial : *#87976633#
3: recovery mode for wiping cache partition and data/factory reset [ Login above or register to see download links. ]

4: redownload your firmware and yoy will have correct modem and CSC files
    regards
